Our Home Care Packages are designed to provide personalised support tailored to your unique needs and preferences, ensuring you receive the assistance necessary to maintain your lifestyle and well-being. We are proud to have a long-standing partnership with Gold Star Home Care and Community Services P/L, a registered Home Care Package provider, further enhancing our commitment to delivering quality care.
Understanding Home Care Packages
Home Care Packages are government-funded programs aimed at supporting older individuals who wish to remain at home but require some assistance with daily activities. These packages offer a coordinated mix of services to help you stay active, healthy, and connected to your community.
How It Works
At Assist Care Management, we understand that every individual has unique needs, which is why Home Care Packages are tailored to provide the right level of support. There are four levels of funding available, depending on the level of care required.
Once your Home Care Package is approved, you can choose a care provider that aligns with your preferences and goals. The services you receive are covered by your package, up to the funding level granted.
To access any Aged Care Services in Australia, including Home Care Packages, you’ll need to register with My Aged Care.

Step 1: Call My Aged Care
Start by calling My Aged Care on 1800 200 422 to apply for a Home Care Package. During the call, you'll provide some basic details about your situation and arrange for an in-home assessment. Have your Medicare card handy, and if someone is calling on your behalf, they'll need your permission to speak on your behalf.
Step 2: Complete the Assessment
A trained assessor will visit your home to understand your care needs and challenges. Be open about the support you require, including tasks you find difficult. This step ensures you receive the right level of care tailored to your needs.
Step 3: Wait for Your Approval Letter
After the assessment, you'll receive a letter confirming your approval and the level of funding you've been granted. This letter also explains the next steps and provides details about your Home Care Package.
Step 4: Begin Your Care with Assist Care Management
Once your package is assigned, you'll receive a referral code that allows us to start your care services. Contact us with your code, and we’ll work with you to create a personalised plan that meets your needs and helps you live independently.

Gardening We can help maintain your outdoor spaces with services like lawn mowing, weeding, and basic garden upkeep.
Laundry From hanging out washing to folding clothes and changing bed linens, we’re here to assist with your laundry needs.
Meal Preparation We’ll help plan and prepare nutritious meals so you can enjoy delicious and balanced meals with ease.
Medication Monitoring Our team provides support to ensure your medications are taken on time and in the correct dosage to keep you healthy.
Personal Assistance Receive respectful and compassionate help with personal care tasks, including bathing, dressing, and grooming.
Respite Care We offer temporary care solutions to give your main carer a well-deserved break, providing planned and reliable respite services.
Social Support & Outings Get help with shopping, attending appointments, or participating in social activities, including transportation to and from these events.
